Birth Certificates
The Charlotte County Tax Collector's Office can now issue certified copies of Florida birth certificates to eligible individuals in our Englewood location.
Obtaining a Birth Certificate
To obtain a birth certificate in our office you must:
1. Meet one of the following criteria:
- Be the child named on the certificate AND 18 years of age or older
- Be the parent listed on the certificate
2. Provide information about the child named on the certificate
- Full name of child at birth and any subsequent legal name changes
- Date of birth
- City or county of birth
- Mother's full name including maiden surname
- Father's full name (if listed on birth record)
3. Provide information about the person ordering the certificate
- Full name
- Relationship to person named on the certificate (father, mother, self, etc.)
- Mailing address
- Daytime contact number
- Signature of applicant
4. Provide valid picture identification. Acceptable forms include:
- Driver license or ID Card
- Passport
- Military ID
- Resident Alien Card
- Certificate of Naturalization
- Inmate ID
- Pilot's License
- Concealed Weapons License
